Form I-693: Report of Immigration Medical Examination and Vaccination Record

Form I-693 is the medical examination and vaccination record required for most adjustment-of-status filings (I-485) and certain other benefits. Only USCIS-designated civil surgeons can complete it. The exam covers communicable diseases, drug abuse, mental disorders with harmful behavior, and required vaccinations.

What Form I-693 Is

Form I-693 is the immigration medical examination report. It documents the results of a physical exam, vaccination review, and required lab tests (tuberculosis, syphilis, gonorrhea) performed by a USCIS-designated civil surgeon.

The civil surgeon completes the form, signs it, seals it in an envelope, and gives it to the applicant. The sealed envelope must be submitted to USCIS — either with the underlying application or at the adjustment interview.

Filing Fee

There is no USCIS filing fee for I-693. The civil surgeon charges out-of-pocket for the medical exam, typically $200–$500. Vaccinations or additional testing may be extra. Some public-health departments offer lower-cost exams.
